Consent to electronic delivery of tax form schedule K-1

This Policy details the types of information we collect from you, or that you provide, when you visit, access, or use our Services. The information we collect may include details you provide through forms on our websites, correspondence records, survey responses, and materials you upload. Different privacy notices may apply to specific services offered through our websites, and we will provide those notices to you when applicable.
- If you do not consent to electronically receive your Schedule K-1s, a paper copy will continue to be mailed to you.
- Unless you withdraw consent, all future Schedule K-1s furnished by Angular Ventures and its associated partnerships will be done so through electronic delivery methods.
- You may request paper copies of your Schedule K-1s by submitting an electronic or paper request to the contact listed under item 10 below. A request for paper copies does not constitute a withdrawal of consent to electronic delivery.
- Consent may be withdrawn by writing to the contact under item 10 below. The withdrawal of consent is effective on the date it is received by Angular Ventures, who will confirm the effective date of the withdrawal in writing.
- A withdrawal of consent will not apply to any Schedule K-1 issued by electronic delivery before the effective date of the withdrawal of consent.
- Fundpanel will continue furnishing electronic Schedule K-1s until a withdrawal of consent is received by Angular Ventures or you cancel your account.
- Any changes in your contact information can be submitted to the address listed under item 10.
- Fundpanel will communicate any change in their contact information by e-mail.
- The Schedule K-1s will be furnished on fundpanelio under the Documents tab. In order to access the website you will need access to the Internet and the Adobe Acrobat Reader software to download, open and view your Schedule K-1s. The Schedule K-1s may be required to be printed and attached to your federal, state or income tax return.
